For those joining the waitlist now, the rates are below. Rates are paid annually. I value continuity of care with my patients and care of the whole family. A reduced rate is available when enrolling your whole household.
Family Rate
$6,775annually
Includes 2 adults and 2 children. Each additional child living in the home is $89. Children involved in the family plan must be under 26 and reside in the home or be in at least half time education matriculation if living outside of the home.
Couple Rate (based on age of oldest member)
$4627 annually for age 19-64
$5630 annually for age 65+
Individual Rates
Age 0-18 years: $2153 annually, without other enrolled adult
Age 19-41 years: $2,479 annually, without other enrolled adult
Age 42-64 years: $3247 annually
Age 65 and up: $3,585 annually
